Client Information Non-DOT Custody Drug Test
This is to inform you that you have requested a non-DOT chain of custody drug test. This type of testing does not meet the Drug Free Workplace Programs that require the test to be performed in a federal licensed labratory as in the case of the Coast Guard, DOT, or FAA modalities. This type of test does meet the state of Hawaii requirements for non-DOT workplace related drug tests.
This account has been established for the convienience of walk in clients who would like to submit a specimen today. it requires a payment up front to proceed and cannot be invoiced.
- Complete all information on the Informed Consent form
- Testing is performed Mondays – Fridays (no testing on holidays or weekends)
- Results will be released to the Medical Review Officer, Dr. Patrick Lam, in 1-3 days. Results are not available from DLS directly
- Dr. Lam’s office will contact you regarding the results. If you have any questions relating to the results, his office address and phone number are printed on your copy of the custody form
